Medications given at
school:

Medications can be given at school if the Doctor and parent follow all procedures:
1. A 135 form must contain written, clear legible, current and unexpired instructions from the student's health care provider regarding the administration of prescribed medications
2. The medication must be furnished in a pharmacy-labeled container with the student's name, the name of the medication, dosage and frequency
3. It is the parent/guardian's responsibility to deliver and maintain an adequate supply of medication at school. The medication may not be delivered by the child
Over the counter medications:
1. A written authorization by the parent must state how the medication is to be dispensed, including the proper dosage as specified by the manufacturer's label. Specific times of the day for dispensation must be provided.
2. A 135 form must contain written, clear, legible, current and unexpired instructions from the health care provider regarding the administration of the medications.
3. The medication must be furnished in a pharmacy-labeled container with the student's name, the name of the medication, dosage and frequency. If the Dr. writes a legible prescription, the medication then can be given without the pharmacy label. All medications have to be in an unbroken manufacturer's original, sealed container. It must be delivered by the parent and not by the child.
